Chandramukhi 2 is a standalone sequel to the 2005 Horror Comedy Telugu-language film Chandramukhi featuring Rajinikanth and Jyothika in the lead roles. The film was a remake of the Kannada-language film Apthamitra (2004), which itself was a remake of the Malayalam film Manichitrathazhu (1993). The 2007 Hindi film Bhool Bhulaiyaa was a remake of Manichitrathazhu (1993).
Jyothika played the titular role of Chandramukhi in the 1993 Tamil-language film, whereas Kangana Ranaut portrays the titular character in Chandramukhi 2 (2023).

Chandramukhi 2 tells the story of a woman who suffers from dissociative identity disorder (DID), a mental health condition where the patient has two or more different personalities. This condition causes her to have multiple characters, creating problems for her family.
To help her, a psychiatrist takes the challenge of solving her case. However, in the process, he puts his own life in danger.
Chandramukhi 2 revolves around how the psychiatrist tries to understand and cure the woman's disorder while facing risks himself.
