Dilip Roy (Rajesh Khanna) is a passionate painter. Sushma (Alka), a rich woman, is his wife. One day, while painting, Sushma asks Dilip to accompany her to a party. Since Dilip is busy painting, he refuses to go with his wife.
It ensures a verbal fight between the couple. Sushma destroys the painting in a rage, and the husband gets emotionally upset and pushes his wife away.
Dilip leaves the house by shouting that he will kill her. Renu (Bindu), the sister of Sushma, witnesses all these from an adjacent room. When he comes back after some time, Dilip finds Sushma dead. Police arrest Dilip on the testimony of Renu.
While undertrial, Dilip becomes mentally unstable. Dr. Trivedi (Gajanan Jagirdar) starts his psychological analysis in the hospital. However, one stormy night Dilip flees from the hospital and lands in Rekha's (Nanda) house, hiding him from the police.
Dilip holds Rekha at gunpoint and orders him not to shout. Rekha informs that her husband is not home. Soon Dilip asks Rekha to forgive him, and they become friendly. Rekha also gives one of her husband's dresses to change the prison uniform.
Believing that Rekha would not call the police, Dilip takes a nap but wakes up at midnight after hearing some noise. Dilip does not find Rekha there and starts searching for Rekha in the entire house. However, he finds the dead body of Rekha's husband in the bathroom.
Rekha appears in the scene, and Dilip starts questioning her. Rekha informs that her husband is in Kolkata and has not returned yet. She also thinks that it is merely the imagination of Dilip. However, Dilip suspects that Rekha is hiding something. Finally, Dilip decides to call the police.
A police team under the supervision of officer Karwe (Iftikhar) reaches there. Dilip reiterates the entire story to the officer. But to his surprise, there is no dead body in the bathroom.
Whether Dilip is hallucinating? If not, how and where the dead body vanishes? Is Rekha hiding something? Further, how did Sushma gets killed? Is it Dilip or someone else? Ittefaq replies to all these queries.
